Responsibilities

• Responsible for the full delivery of population interventions, including assessment, individualized wellness strategies, health enhancement and health maintenance strategies.
• Perform patient wellness tests, clinical assessments and evaluations.
• Design, execute, and evaluate safe, effective wellness programs for individuals and groups.
• Apply current knowledge, theory, evidence and sound clinical judgment in patient management.
• Review, analyze, and document patient results in clinical documentation tools.
• Assist with department and program planning.
• Research best practices, measure effectiveness and performance.
• Participate as member of collaborative research teams that examine relationships among nutrition, physical activity, resiliency and overall health.
• Understand and promote the Healthy Living Program philosophy and assist patients with establishing an ongoing relationship with Mayo Clinic.

Qualifications

• Licensure or eligible for licensure in the state of Minnesota.
• Advanced degree in a health‑care field.
• Board certification in clinical specialty area.
• Active participation within professional organization.
• Demonstrated coaching skills including reflective listening and motivational interviewing.
• Excellent customer service skills to foster a premium patient experience.
• Physical therapy licensure in the state of Minnesota.
• Limited Scope X‑ray Operator certification through the Minnesota Department of Health within six months from hire date.
• Basic Life Support certification.

Compensation

$68,373.76 – $102,602.24 / year

Schedule

Full time; Monday, Wednesday, Thursday, and Friday; 64 hours per week.
