Job summary

Spectrum Retirement Communities is seeking a part-time Wellness Nurse to support residents with clinical monitoring and care coordination. The role involves assisting with admissions, administering medications, and collaborating with nurses and providers to ensure regulatory compliance and excellent care.

The position requires nursing licensure (LPN or RN), state registration, and at least 3 years of geriatric assessment experience. A compassionate, team-oriented mindset is essential.

Qualifications

• Must comply with state regulations, policies and procedures.
• Graduate of an accredited practical nursing program.
• Current LPN/RN license and state registration.

Responsibilities

• Coordinate resident admissions including screening, assessment and care needs.
• Deliver care per resident care plans.
• Administer medications and treatments per physician orders.
• Coordinate care with outside providers and resident physicians.
• Document care and resident observations; report significant changes.
• Maintain knowledge of state regulations, policies and procedures.

Job Description

Schedule: Part-Time, 7am-7pm.

The Wellness Nurse supports the health and well-being of residents through ongoing clinical monitoring and support, provides assistance to community nurses, caregivers, and medication personnel to help ensure regulatory compliance and clinical excellence. The ability to work effectively as part of a team with flexibility and adaptability to change and pace of duties while maintaining a warm and encouraging disposition is important.

• Assist in coordination of the resident admission process including screening, assessment, and care needs
• Ensure care is delivered as indicated in resident care plans
• Medication and treatment administration and ensure medications are administered per physicians’ orders
• Assistance with coordinating care and services with other outside providers including resident physicians
• Document and report care provided and resident observations including unusual or significant changes
• Maintain a working knowledge of state regulations, policies, and procedures

Qualifications

To be successful in this position, we believe that you need the following experiences, strengths, and skills:

• Working knowledge of state regulations, policies, and procedures, dictated for compliance
• Graduate from an accredited school of practical nursing
• Current nursing license and state registration (LPN or RN)
• Three years' experience in assessment and geriatric nursing, preferred
• A big smile, eagerness to learn, and compassion for our Residents
