Job summary

Spectrum Retirement Communities, LLC is seeking a part-time Wellness Nurse to support residents' health through clinical monitoring and coordination with nurses, caregivers, and medication staff. Qualifications

• Knowledge of state regulations, policies, and procedures applicable to nursing.
• Graduate from an accredited school of practical nursing and valid nursing license.
• Licensed LPN or RN with state registration and ongoing professional practice.
• Experience in geriatric assessment and nursing is preferred.

Responsibilities

• Coordinate resident admission through screening and assessment and care needs.
• Ensure care is delivered per resident care plans and physician orders.
• Administer medications and treatments according to orders.
• Coordinate care with outside providers including residents' physicians.
• Document care provided and resident observations, noting significant changes.
• Maintain knowledge of state regulations, policies, and procedures.

Job Description

Schedule: Part-Time. Day Shift.

The Wellness Nurse supports the health and well-being of residents through ongoing clinical monitoring and support, provides assistance to community nurses, caregivers, and medication personnel to help ensure regulatory compliance and clinical excellence. The ability to work effectively as part of a team with flexibility and adaptability to change and pace of duties while maintaining a warm and encouraging disposition is important.

• Assist in coordination of the resident admission process including screening, assessment, and care needs
• Ensure care is delivered as indicated in resident care plans
• Medication and treatment administration and ensure medications are administered per physicians’ orders
• Assistance with coordinating care and services with other outside providers including resident physicians
• Document and report care provided and resident observations including unusual or significant changes
• Maintain a working knowledge of state regulations, policies, and procedures

Qualifications

To be successful in this position, we believe that you need the following experiences, strengths, and skills:

• Working knowledge of state regulations, policies, and procedures, dictated for compliance
• Graduate from an accredited school of practical nursing
• Current nursing license and state registration (LPN or RN)
• Three years’ experience in assessment and geriatric nursing, preferred
