Job summary

Bridge Senior Living LLC is seeking a dedicated Wellness Nurse to coordinate and deliver high-quality care for our senior residents. You will work collaboratively with physicians and care teams to ensure exceptional care and a warm, welcoming environment.

Ideal candidates will have weekend availability, an LPN license, and CPR certification, with at least one year of related experience. We offer competitive pay, health benefits, and opportunities for growth.

Qualifications

• High school diploma or equivalent (GED).
• LPN license in applicable state.
• First Aid & CPR certification.
• At least 1 year of related work experience.

Responsibilities

• Assessment, planning, implementation, and evaluation of residents.
• Provide high level of care by frequent rounds and documenting care.
• Assist the Director of Wellness in care plan development and updates.
• Maintain communication with residents, families, and physicians.

Wellness Nurse Job Summary:

Wellness Nurse will implement and coordinate the delivery of care in collaboration with physicians and care associates. Above all, you will exhibit a passion for serving seniors and contributing to the positive environment our residents are proud to call home.

• Assessment, planning, implementation, and evaluation of our residents
• Provide high level of care by making frequent rounds, ensuring continuity of care, documenting all pertinent care information, and observing our residents' overall condition and behavior.
• Assist the Director of Wellness in the development and update of resident care plans.
• Maintain ongoing communication with residents, families, and physicians regarding the resident’s care.
