Bridge Senior Living LLC in Florida is seeking a dedicated Wellness Nurse to join our team on Fridays and Saturdays from 7a-7p. You will deliver coordinated care in collaboration with physicians and care staff, while supporting a compassionate, resident‑centered environment.

The ideal candidate has an LPN license in Florida, a high school diploma or GED, First Aid & CPR certification, and at least one year of related experience.

Qualifications

• High school diploma or GED required.
• LPN license in applicable state.
• First Aid & CPR certification.
• At least 1 year of related work experience.

Responsibilities

• Coordinate and deliver care in collaboration with physicians and care staff.
• Make frequent rounds, document care and monitor residents' condition.
• Assist in developing and updating resident care plans.
• Maintain ongoing communication with residents, families, and physicians.

Wellness Nurse Job Summary:

Wellness Nurse will implement and coordinate the delivery of care in collaboration with physicians and care associates. Above all, you will exhibit a passion for serving seniors and contributing to the positive environment our residents are proud to call home.

• Assessment, planning, implementation, and evaluation of our residents
• Provide high level of care by making frequent rounds, ensuring continuity of care, documenting all pertinent care information, and observing our residents' overall condition and behavior.
• Assist the Director of Wellness in the development and update of resident care plans.
• Maintain ongoing communication with residents, families, and physicians regarding the resident’s care.
