Job summary

Spectrum Retirement Communities is seeking a Wellness Nurse (LPN/RN) in Lombard, IL to provide clinical support for residents on a NOC PRN schedule. The role requires an active nursing license and experience in geriatric care.

The Nurse will assist with medications, care plans, and regulatory compliance while collaborating with the team. The position offers hourly pay between $32.75 and $36.20, commensurate with experience, and includes eligibility for comprehensive health, dental, and vision

Qualifications

• State regulations knowledge for compliance.
• Graduate of practical nursing program.
• Current LPN or RN license and registration.
• Three years’ experience in geriatric assessment preferred.
• Compassion and teamwork.

Responsibilities

• Coordinate resident admission processes including screening and assessment.
• Deliver care per resident care plans and monitor changes.
• Administer medications and treatments per physician orders.
• Coordinate with physicians and outside providers for resident care.
• Document care provided and observe resident changes.
• Maintain knowledge of state regulations and procedures.

Job Description

Schedule: NOC PRN. Must work at minimum one shift per pay period.

Salary: $32.75-36.20 per hour, commensurate with experience.

The Wellness Nurse supports the health and well-being of residents through ongoing clinical monitoring and support, provides assistance to community nurses, caregivers, and medication personnel to help ensure regulatory compliance and clinical excellence. The ability to work effectively as part of a team with flexibility and adaptability to change and pace of duties while maintaining a warm and encouraging disposition is important.

• Assist in coordination of the resident admission process including screening, assessment, and care needs
• Ensure care is delivered as indicated in resident care plans
• Medication and treatment administration and ensure medications are administered per physicians’ orders
• Assistance with coordinating care and services with other outside providers including resident physicians
• Document and report care provided and resident observations including unusual or significant changes
• Maintain a working knowledge of state regulations, policies, and procedures

Qualifications

To be successful in this position, we believe that you need the following experiences, strengths, and skills:

• Working knowledge of state regulations, policies, and procedures, dictated for compliance
• Graduate from an accredited school of practical nursing
• Current nursing license and state registration (LPN or RN)
• Three years’ experience in assessment and geriatric nursing, preferred
