Job summary

Spectrum Retirement Communities seeks a Wellness Nurse for PRN shifts in our Illinois community. The role focuses on clinical monitoring, supporting community nurses, and ensuring regulatory compliance while delivering compassionate care to residents.

Qualifications include a practical nursing credential (LPN/RN) with state registration and at least three years in geriatric assessment. A team-oriented, caring attitude is essential to thrive in this role.

Qualifications

• Knowledge of state regulations, policies, and procedures for nursing.
• Graduate from an accredited school of practical nursing.
• Current LPN or RN license and state registration.
• Three years’ experience in assessment and geriatric nursing (preferred).

Responsibilities

• Coordinate resident admission process including screening, assessment, and care needs.
• Ensure care is delivered as indicated in resident care plans.
• Administer medications and treatments per physician orders.
• Coordinate care with outside providers including resident physicians.
• Document and report care provided and resident observations.
• Maintain knowledge of state regulations, policies, and procedures.

Schedule: PRN All Shifts, must work at minimum, one shift per pay period

The Wellness Nurse supports the health and well-being of residents through ongoing clinical monitoring and support, provides assistance to community nurses, caregivers, and medication personnel to help ensure regulatory compliance and clinical excellence. The ability to work effectively as part of a team with flexibility and adaptability to change and pace of duties while maintaining a warm and encouraging disposition is important.

• Assist in coordination of the resident admission process including screening, assessment, and care needs
• Ensure care is delivered as indicated in resident care plans
• Medication and treatment administration and ensure medications are administered per physicians’ orders
• Assistance with coordinating care and services with other outside providers including resident physicians
• Document and report care provided and resident observations including unusual or significant changes
• Maintain a working knowledge of state regulations, policies, and procedures

• Working knowledge of state regulations, policies, and procedures, dictated for compliance
• Graduate from an accredited school of practical nursing
• Current nursing license and state registration (LPN or RN)
• Three years’ experience in assessment and geriatric nursing, preferred
