Job summary

The Neighborhood at Paducah is seeking a Wellness Nurse/LPN to join our team in Paducah, KY. The LPN reports to the Wellness Director and supports resident health, wellness initiatives, and care coordination.

Responsibilities include medication administration, resident assessments through PSP, and collaboration with families and support services. Weekend availability and leadership demonstrated in prior roles are preferred.

Qualifications

• Licensed Practical Nurse or Registered Nurse.
• Minimum of one year experience with elders or disabled individuals preferred.
• Demonstrated leadership capabilities.
• Knowledge of Assisted Living or Dementia care preferred.
• Ability to work weekends, evenings and flexible hours.

Responsibilities

• Identifies ongoing needs and services of residents through the assessment/Personalized Service Plan (PSP) process.
• Ensures proper follow through and documentation for residents with changes in clinical care needs.
• Assist Wellness Director with completing the clinical sections of monthly wellness visits and PSP to accurately reflect the resident monthly status.
• Inform Wellness/Executive Director of changes in medication or service level impacting tier level or move-out potential.
• Assist with obtaining weights and vital signs monthly for each resident prior to monthly wellness health updates.
• Assist with direct resident care as needed.
