Job summary

Oakmont Management Group seeks a Wellness Nurse to coordinate resident care and medication management under physician supervision, ensuring residents stay as independent as possible seven days a week.

Responsibilities include communicating with healthcare professionals, coordinating multiple care services, administering meds, and documenting thoroughly. California RN/LVN license required, CPR/First Aid, and background checks are mandatory.

Qualifications

• Must be a Licensed Vocational or Registered Nurse with a California license in good standing.
• Prefer one year of experience in a Residential Care Facility for the Elderly (RCFE).
• Proficient in MS Office programs (Word, Excel, etc.).
• Able to operate standard office equipment including fax, copier, telephone, etc.
• Knowledge of assisted living and dementia care.

Responsibilities

• Communicate with healthcare professionals including physicians, discharge planners, nurses, and state agencies on residents’ care needs.
• Coordinate incidental medical, dental, vision, hearing, and podiatry care for residents.
• Administer medications and perform treatments as ordered by a physician.
• Provide support to Medication Technicians to ensure competent medication delivery.
• Assist with resident assessment tasks as directed by the Health Services Director.
• Conduct staff interviews and monitor residents to keep staff informed of health changes.
• Document thoroughly in the resident chart per policy and regulations.
• Review Shift Reports and participate in handoff meetings with pertinent information.
• Coordinate prescription orders with doctors and pharmacy delivery; oversee storage and delivery of meds.
• Communicate with residents and families about preferred pharmacy and incontinence supply programs.
