The Wellness Nurse at Oakmont Management Group is a licensed nurse responsible for coordinating resident care and medication management under medical direction. The role ensures residents stay as independent as possible, seven days a week, with coverage when Health Services Director is off.

Responsibilities include clinical coordination, medication administration, documentation, and collaboration with physicians, discharge planners, and nursing staff to maintain high-quality care across Oakmont

Formación

• Must hold a valid California LVN or RN license.
• Prefer 1 year of RCFE experience.
• Proficient with MS Office (Word, Excel).
• Able to operate standard office equipment (fax, copier, phone).
• Knowledge of assisted living and dementia care.

Responsabilidades

• Communicate with healthcare professionals on resident care needs.
• Coordinate incidental medical and vision/hearing care for residents.
• Administer medications and support treatments as directed by physicians.
• Support Medication Technicians in delivering medications.
• Assist with resident assessments as directed by Health Services Director.
• Conduct staff interviews and communicate changes to physicians and team.
• Complete documentation in resident charts per policy and regulations.
• Monitor shift reports and participate in handovers.
• Coordinate prescription orders with doctors and pharmacies.
• Communicate with residents and families regarding pharmacy and incontinence supply programs.
