Job summary

The Aspenwood Company seeks a compassionate Wellness Nurse at Spring Creek Village in Spring, TX to provide high-quality, person-centered care. You’ll administer medications, supervise aides, and coordinate with the care team to uphold dignity and safety.

Full-time, on-site role requiring LVN/LPN license and BLS/CPR, familiarity with EHR, and HIPAA compliance. Weekend/holiday availability. Pay rate is $28.00/hr with comprehensive benefits.

Qualifications

• Licensed nurse (LVN/LPN) with current license.
• Experience with assisted living or memory care settings is a plus.
• Familiarity with HIPAA and patient privacy standards.

Responsibilities

• Administer medications safely per physician orders.
• Supervise medication aides and conduct monthly reconciliations.
• Coordinate daily staff assignments to ensure consistent resident care.
• Communicate with physicians, families, and care teams.
• Provide direct care including ADLs and mobility assistance.
• Document assessments and service plans accurately.
• Ensure compliance with regulatory standards.

Location: Spring Creek Village, an Independent and Assisted Living with Memory Care senior community

Pay Rate: $28.00/hr.

Full Time Schedule: 6:00a.m. - 2:00p.m.

Must be available to work weekends and holidays.

Essential Duties and Responsibilities:

• Administer medications safely and accurately per physician orders and regulatory standards
• Oversee medication aides and conduct monthly medication reconciliation and audits
• Coordinate daily staff assignments to ensure consistent resident care
• Communicate effectively with physicians, families, and care teams regarding resident needs
• Provide direct care including ADLs: bathing, dressing, grooming, toileting, mobility, and incontinence support
• Respond to medical and behavioral emergencies with sound clinical judgment
• Complete clinical documentation including assessments, evaluations, and service plans
• Document incidents and communicate promptly with leadership
• Monitor care team documentation for accuracy and completeness
• Support orientation and ongoing training of care staff
• Assist with scheduling in collaboration with leadership
• Attend required in-services and continuing education
• Maintain professional appearance and hygiene-
• Promote respectful and effective communication across teams and families
• Perform other duties assigned to support resident care and operational excellence

Qualifications:

• Current and valid LVN/LPN license in the state of employment
• Basic Life Support (BLS) / CPR certification
• Minimum 6 months to 1 year experience in Assisted Living or Long-Term Care
• Proficient in Electronic Health Records (EHR) and basic computer skills
• Strong verbal and written communication skills
• Demonstrated critical thinking and problem-solving abilities
• Knowledge of HIPAA regulations and compliance standards
• Ability to respond with patience and understanding to dementia-related behaviors
• Physical ability to perform essential job functions with or without reasonable accommodation
