Job summary

Spring Creek Village is seeking a compassionate Wellness Nurse to administer medications, oversee resident care, and support a team dedicated to dignity, safety, and person-centered service.

This full-time role requires LVN/LPN licensure in Texas, BLS/CPR, and experience in assisted living or long-term care; you will work 6:00 a.m. - 2:00 p.m., weekends and holidays, with benefits and opportunities for professional growth.

Qualifications

• Current and valid LVN/LPN license in the state of employment.
• BLS/CPR certification.
• Minimum 6 months to 1 year experience in Assisted Living or Long-Term Care.
• Proficient in Electronic Health Records (EHR) and basic computer skills.
• Strong verbal and written communication skills.
• Demonstrated critical thinking and problem-solving abilities.
• Knowledge of HIPAA regulations and compliance standards.
• Ability to respond with patience and understanding to dementia-related behaviors.
• Physical ability to perform essential job functions with or without reasonable accommodation.

Responsibilities

• Administer medications safely and accurately per physician orders.
• Oversee medication aides and conduct monthly medication reconciliation and audits.
• Coordinate daily staff assignments to ensure consistent resident care.
• Communicate effectively with physicians, families, and care teams regarding resident needs.
• Provide direct care including ADLs: bathing, dressing, grooming, toileting, mobility, and incontinence support.
• Respond to medical and behavioral emergencies with sound clinical judgment.
• Complete clinical documentation including assessments, evaluations, and service plans.
• Document incidents and communicate promptly with leadership.
• Monitor care team documentation for accuracy and completeness.
• Support orientation and ongoing training of care staff
• Assist with scheduling in collaboration with leadership.
• Attend required in-services and continuing education.
• Maintain professional appearance and hygiene-
• Promote respectful and effective communication across teams and families
• Perform other duties assigned to support resident care and operational excellence

Location: Spring Creek Village, an Independent and Assisted Living with Memory Care senior community

Pay Rate: $28.00/hr.

Full Time Schedule: 6:00a.m. - 2:00p.m.

Must be available to work weekends and holidays.
