Job summary

Senior Living Residences, LLC's Ridgefield Station in Ridgefield, CT is seeking a Wellness Nurse (LPN) to support the Resident Care Director by developing and supervising a comprehensive care program for residents.

You will train and manage CNAs, coordinate with families, physicians, and community agencies, and contribute to a high-quality, person-centered living environment with a strong focus on dementia care and brain healthy activities.

Qualifications

• CT license required (LPN or RN).
• 1+ year experience with elderly and/or Alzheimer's populations.
• Excellent communication with residents, families, and physicians.

Responsibilities

• Assist with managing and developing a comprehensive care program for all residents.
• Train and manage CNAs and coordinate with families and physicians.
• Serve as liaison with family members, community agencies, physicians and care providers.
