Job Overview

The Wellness Nurse monitors the health and well-being of residents, provides leadership support, and assists with resident care and services. Responsibilities include ensuring resident well-being, training and monitoring medication administration, communicating with support services and families, and maintaining clinical quality assurance, all in accordance with standards and policies to promote the highest quality of care.

Responsibilities & Qualifications
Essential Duties

Supporting our Mission, Principles of Service, and Core Values, with a focus on the sacred value of human life. Key responsibilities include:

Coordination of Health Needs
  • Identify ongoing resident needs and services to ensure high-quality care.
  • Follow up and document changes in resident health status.
  • Assist in evaluations and data collection during move-in, 30-day, semi-annual, and change of status assessments.
  • Conduct monthly wellness visits.
  • Monitor and document incidents like falls or accidents.
  • Maintain communication with residents’ families and physicians.
  • Ensure weights and vital signs are obtained monthly.
  • Maintain wellness files and medical supplies.
  • Provide clinical support to team members and follow infection control practices.
Medication Programs
  • Monitor medication and treatment profiles for accuracy.
  • Transcribe physician orders accurately.
  • Complete monthly MAR/TAR audits.
  • Ensure proper medication administration rights.
  • Coordinate pharmacy services and maintain medication areas.
Quality Assurance & Compliance
  • Participate in quality improvement initiatives and meetings.
  • Ensure compliance with regulations and resident rights.
  • Review physician reports and order medications/equipment as directed.
  • Contribute to resident care programs and documentation.
  • Engage in team and community meetings, training, and compliance activities.
Core Competencies
  • Ability to handle multiple priorities effectively.
  • Strong communication, organizational, and judgment skills.
Experience & Qualifications
  • Graduate of approved nursing program.
  • Current state/provincial LPN/LVN license.
  • At least one year of experience in healthcare settings.
  • Knowledge of nursing practices, regulations, and assessment skills.
  • Proficiency in computer applications and Sunrise systems.
  • Flexibility to work weekends, evenings, and variable hours.
