Job summary

The Pearl at Jamestown is seeking a Wellness Nurse/LPN to join their team in Baton Rouge, LA. The role reports to the Wellness Director and covers 12-hour day and night rotating shifts, providing medication administration and resident care with a focus on wellness and quality assurance.

Responsibilities include identifying resident needs through PSPs, documenting changes, and supporting the Wellness Director.

Qualifications

• Must have demonstrated Leadership capabilities.
• Must be a Licensed Practical Nurse (LPN) or Registered Nurse.
• Minimum of one year of experience working with elders or disabled individuals.

Responsibilities

• Monitors residents' health and wellbeing and provides leadership support with resident care and services.
• Administers medications and communicates with resident support services and families.
• Maintains clinical quality assurance while leading wellness initiatives.
• Identify ongoing needs and services through the Personalized Service Plan (PSP) process.
• Ensure proper follow-through and documentation for changes in clinical care needs.
• Assist Wellness Director with monthly wellness visits and PSP updates.
• Record weights and vital signs monthly and maintain wellness health updates.
• Provide direct resident care as needed.
