Job summary

Responsibilities

• Assess, plan, implement, and evaluate resident care.
• Provide high level of care by making frequent rounds, documenting pertinent care information and observing residents’ condition.
• Assist the Director of Wellness in developing and updating resident care plans.
• Maintain ongoing communication with residents, families and physicians regarding care.
