Base pay range: $30.30/hr - $40.45/hr

• Identify ongoing needs of residents to promote high-quality services.
• Participate in evaluations and document changes in residents’ health status.
• Conduct monthly wellness visits and follow up on incidents.
• Maintain communication with residents’ families and healthcare providers.
• Monitor and document medication and treatment profiles.
• Support infection control practices and regulatory compliance.
• Assist in maintaining wellness files and medical supplies.
• Provide clinical support to team members.

Qualifications

• Graduate of approved nursing program.
• Current state license as LPN/LVN.
• Minimum one year of experience in healthcare settings.
• Knowledge of nursing practices, regulations, and assessment skills.
• Effective communication, organizational, and decision-making skills.
• Proficiency in computer skills and Sunrise applications.
• Availability to work weekends, evenings, and flexible hours.
