Job summary

Silver Birch Living, LLC. is seeking a Wellness Nurse (LPN or RN) to join the Clinical Team at Silver Birch Fort Wayne North. You will support residents’ health, safety, and overall well-being through medication management, assessments, and daily clinical support.

The role emphasizes compassionate care, teamwork, and education for residents and families. A current LPN/RN license, CPR, TB, and First Aid certifications are required, with at least two years in senior living and a high school

Qualifications

• High school diploma or GED required.
• Two+ years of experience in senior living.
• Active LPN or RN license in good standing.
• Current CPR, TB, and First Aid certifications.

Responsibilities

• Administer medications and treatments with accurate documentation.
• Complete resident assessments including admissions, transfers, discharges, and changes in condition.
• Monitor resident health including vital signs and post-incident follow-ups.
• Educate residents and families on care plans and wellness needs.
• Support and guide QMAs and CNAs during each shift.
• Collaborate with providers and interdisciplinary teams to ensure high-quality care.

What You’ll Be Doing

What We’re Looking For

• A high school diploma or GED
• Two (2) or more years of experience working in senior living
• A current LPN or RN license in good standing
• Valid CPR, TB, and First Aid certifications
• A nurse who is organized, compassionate, and comfortable working as part of a team
