Job summary

Plush Mills Senior Living invites you to join our Wellness Nurse (LPN) team, delivering high-quality care to residents and supporting daily wellness routines.

You will administer medications, supervise care staff, track care plans, and respond to health changes while upholding dignity and safety. This role requires an active LPN license in the state of Pennsylvania and a compassionate, patient-centered approach; shift is 10pm–6am to support overnight care.

Qualifications

• Active LPN license in the state of employment.
• Minimum 1 year of nursing experience; Alzheimer's/dementia care experience preferred.
• Strong communication, interpersonal, and leadership skills.
• Annual flu vaccination required.

Responsibilities

• Provide direct nursing care and supervise medication care specialists.
• Administer medications, treatments, and procedures per physician’s orders.
• Document resident care and update care plans as needed.
• Monitor for changes in residents’ health and communicate updates to physicians and families.
• Assist with resident move-ins and orientation.
• Provide leadership and guidance to care staff, including performance evaluations and coaching.
• Participate in emergency response procedures and ensure safety standards are upheld.
• Maintain accurate resident records and ensure compliance with community policies and state regulations.
• Promote resident independence, wellness, and participation in daily activities.
• Foster a positive, respectful environment for residents, families, and team members.

Wellness Nurse (LPN) Role Overview:
