Job summary

Plush Mills in Pennsylvania is seeking a Wellness Nurse (LPN) to deliver high-quality care to our senior residents. You will administer medications, support daily wellness routines, supervise care staff, and ensure residents’ health and dignity.

Active LPN license in PA and a minimum of 1 year of nursing experience are required, with Alzheimer’s/dementia experience preferred. Strong communication and leadership skills, plus a commitment to compassionate care, are essential.

• Document resident care and update care plans as needed.
• Monitor for changes in residents’ health and communicate updates to physicians and families.
• Assist with resident move-ins and orientation.
• Provide leadership and guidance to care staff, including performance evaluations and coaching.
• Participate in emergency response procedures and ensure safety standards are upheld.
• Maintain accurate resident records and ensure compliance with community policies and state regulations.
• Promote resident independence, wellness, and participation in daily activities.
• Foster a positive, respectful environment for residents, families, and team members.
