Job summary

Spectrum Retirement Communities is seeking a Wellness Nurse to support residents' health through clinical monitoring and collaboration with nurses, caregivers, and medication personnel. This full-time role requires teamwork, flexibility, and a warm, encouraging bedside manner.

Responsibilities include coordinating admissions, ensuring care aligns with plans, administering medications, coordinating with physicians, documenting care, and staying current with state regulations in Ohio.

Qualifications

• Knowledge of state regulations, policies, and procedures
• Graduate from an accredited school of practical nursing
• Current nursing license and state registration (LPN or RN)
• Three (3) years'experience in assessment and geriatric nursing
• Compassion, eagerness to learn, and warmth with residents

Job Description

Schedule: Full-Time

The Wellness Nurse supports the health and well-being of residents through ongoing clinical monitoring and support, provides assistance to community nurses, caregivers, and medication personnel to help ensure regulatory compliance and clinical excellence. The ability to work effectively as part of a team with flexibility and adaptability to change and pace of duties while maintaining a warm and encouraging disposition is important.

In this position, your main responsibilities will include:

• Assist in coordination of the resident admission process including screening, assessment, and care needs
• Ensure care is delivered as indicated in resident care plans
• Medication and treatment administration and ensure medications are administered per physicians’ orders
• Assistance with coordinating care and services with other outside providers including resident physicians
• Document and report care provided and resident observations including unusual or significant changes
• Maintain a working knowledge of state regulations, policies, and procedures

Qualifications

To be successful in this position, we believe that you need the following experiences, strengths, and skills:

• Working knowledge of state regulations, policies, and procedures, dictated for compliance
• Graduate from an accredited school of practical nursing
• Current nursing license and state registration (LPN or RN)
• Three (3) years'experience in assessment and geriatric nursingpreferred
