Spectrum Retirement Communities is seeking a part-time Wellness Nurse to support residents' health and well-being. You will assist with admissions, care plans, medication administration, and coordination with physicians and nurses in a team-focused, compassionate environment.

The role requires an LPN or RN license with state registration and prior geriatric assessment experience. You will work a 7am-3pm shift, every other weekend, with good teamwork and regulatory compliance as core duties.

Qualifications

• Graduate from an accredited school of practical nursing.
• Current nursing license and state registration (LPN or RN).
• Three (3) years of experience in assessment and geriatric nursing preferred.
• Knowledge of state regulations and procedures for compliance.
• Compassion and ability to work well in a team.

Responsibilities

• Assist in coordination of the resident admission process including screening, assessment, and care needs.
• Ensure care is delivered as indicated in resident care plans.
• Medication and treatment administration and ensure medications are administered per physicians’ orders.
• Coordinate care and services with outside providers including resident physicians.
• Document and report care provided and resident observations including unusual or significant changes.
• Maintain a working knowledge of state regulations, policies, and procedures.

Job Description

Schedule: Part-Time. Shift: 7am-3pm,every other weekend.

The Wellness Nurse supports the health and well-being of residents through ongoing clinical monitoring and support, provides assistance to community nurses, caregivers, and medication personnel to help ensure regulatory compliance and clinical excellence. The ability to work effectively as part of a team with flexibility and adaptability to change and pace of duties while maintaining a warm and encouraging disposition is important.
