Responsibilities

• Department Leadership & Daily Operations: Oversee planning, developing, organizing, implementing, evaluating, and directing the day-to-day functions of Wellness Hub.
• Villager Care & Communication: Communicate with Villagers and families regarding all care; review incidents and 24-hour reports; participate in meetings.
• Medication Management & Coordination: Review and reconcile medication errors; liaise with pharmacy/provider; support Self Administration of Medication program.
• Supplies & Inventory Management: Ensure stock of medications, supplies, and equipment; notify personnel of needs.
• Other Responsibilities: Perform other duties to meet residents' needs.

Position Purpose

The Wellness Hub Manager is responsible for overseeing the day-to-day operations of the Wellness Hub department, ensuring that all departments and processes work seamlessly together. This role is crucial in maintaining the smooth functioning of the Personal Care Homes, ensuring high-quality care for Villagers, and promoting a collaborative environment among staff. The manager ensures operational efficiency, supports staff coordination, and addresses any issues that arise to uphold the highest standards of health and safety.

Major Duties and Responsibilities

Department Leadership & Daily Operations

• Oversee planning, developing, organizing, implementing, evaluating, and directing the day-to-day functions of the Wellness Hub department.
• Participate in the Wellness Hub department.
• Evaluates work performance of Wellness Hub Professionals and Wellness Hub Villager Appointment Coordinator and provided performance coaching according to operational policies and as directed.

Villager Care & Communication

• In collaboration with the Personal Care Home (PCH) Senior Nurse Manager communicate with Villagers and Villager families regarding all care.
• Reviews all incidents and accidents and the 24-hour report daily to ensure prompt follow-up by appropriate team members.
• Participate in Program and Services Meeting (P&S).
• Participate in PCH updates

Medication Management & Coordination

• Review and reconcile medication errors daily.
• Serves as the primary liaison to the pharmacy and provider group.
• Assist the Personal Care Home (PCH) Senior Nurse Manager with the Self Administration of Medication program.
• Complete processing of medication order, re-order, delivery, and verification processes.

Supplies & Inventory Management

• Ensures operating and medical supplies are ordered and stocked including incontinence products, office supplies, wound care supplies.
• Ensures that there is adequate stock of medications, supplies, and equipment and notifies appropriate personnel of needs.

Other Responsibilities

• Performs other duties that may be necessary in the best interest of Annandale at Suwanee, Inc. to meet the personal service and care needs of the facility residents.

Required Qualifications

Employment Requirements

• Must be at least 21 years of age, have a valid Class C Georgia Driver’s License, proof of vehicle insurance, and meet company policy regarding MVR requirements.
• Must meet state requirements regarding a GBI and FBI criminal history record check which requires fingerprinting.
• Must undergo and pass a drug screening and screening for Tuberculosis.
