Job summary

NIVA Health is seeking a Wellness Case Manager to guide patients through their wellness journey. You will engage with patients from their first clinic interaction through ongoing programme participation, building trust and delivering exceptional service.

The role blends relationship-building, patient support, organization, and measurable performance targets in a fast-paced, people-focused environment. On-site clinic work is required.

Qualifications

• Empathy and client-focused communication required.
• Experience in patient engagement or healthcare coordination is preferred.
• Strong organizational skills and ability to manage multiple interactions.

Responsibilities

• Build trust-based relationships with patients.
• Support patient engagement and programme participation.
• Guide patients through provider-directed services and next steps.
• Coordinate follow-ups, scheduling, and communications.
• Help create a seamless, elevated patient experience.
• Track KPIs related to engagement and follow-up.
