Job summary

Brightli is seeking a Disease Management Specialist in Independence, MO to provide outreach, engagement, care coordination, and resource navigation for individuals with mental health and substance use needs.

You will use motivational interviewing and person-centered approaches to connect clients with behavioral health and community resources, while collaborating closely with intake teams and healthcare partners.

Qualifications

• Bachelor's degree in Social Work, Criminal Justice, Counseling, Psychology, or related field.
• Minimum of two years in a mental health or related human services setting.
• Knowledge of the judicial system preferred.

Responsibilities

• Engage individuals identified through the Disease Management Program using motivational interviewing and person-centered practices.
• Coordinate admissions and referrals with intake teams to ensure timely access to services.
• Maintain accurate program documentation and compliance with state reporting requirements.
• Collaborate with Healthcare Home staff to promote wellness education and preventative care.
