Vitality Living Franklin is seeking a Wellness Director to lead clinical oversight and resident care on site. You will supervise wellness staff, complete nursing assessments, develop and review care plans with physician orders, and coordinate services with home health and hospice to ensure timely, high‑quality care for residents.

Qualifications

• LPN/LVN/RN license required.
• At least 2 years of experience as a Director in Assisted Living or Memory Care.
• Professional leadership and communication skills.

Responsibilities

• Directly supervise wellness staff with hiring, training, and evaluations.
• Complete nursing assessments and resident evaluations.
• Oversee care plan establishment and reviews with families and physician orders.
• Monitor MAR/eMAR and maintain accurate resident data.
• Coordinate services with home health, hospice, and other agencies.
• Occasionally fulfill charge nurse duties to evaluate systems and care.
