Belmont Village Rancho Santa Fe, a newly constructed luxury Assisted Living, Memory Care and Independent Living community, seeks a Wellness Coordinator to oversee clinical and care operations and support the Director of Nursing in wellness initiatives and quality monitoring.

Responsibilities

• Coordinate day-to-day operations of the Wellness Center.
• Follow physician orders and coordinate services with providers.
• Administer medications and monitor for adverse reactions.
• Ensure care plans are implemented by staff.
• Maintain resident medical records and incident reports.
• Assist in regulatory compliance and resident satisfaction efforts.

ABOUT THE ROLE

As a Wellness Coordinator at Belmont Village, you will oversee and manage clinical and care aspects of the community while supporting the Director of Nursing in wellness initiatives, regulatory compliance and quality monitoring. You must be a licensed nurse – LVN, LPN or RN. You will communicate with healthcare practitioners, ancillary providers, pharmacies, family members, and team members as needed ensuring timely clinical interventions and follow-up. You will lead and provide direction to Caregivers and Medication Technicians where applicable.

YOUR TYPICAL RESPONSIBILITIES

• Coordinate day-to-day operations of the community Wellness Center

• Follow up on physicians orders, coordinate services with ancillary providers and report to physicians and families any changes of condition or other pertinent clinical care information

• Administer medication to an assigned group of residents according to physicians orders and report any adverse reactions or clinical observations documenting as necessary

• Ensure resident care and service plans are followed by supervising Medication Technicians and Caregivers

• Make appropriate situational decisions and manage conflict resolution constructively

• Update and review resident medical records, clinical documentation, and incident reports

• Assist the Director of Resident Care Services in regulatory compliance and enhancing resident satisfaction

• Assist in maintaining and training of infection control and hazard communication programs

MINIMUM QUALIFICATIONS

• Active LVN or LPN nursing license in the State in which this community is located

• CPR Certified or ability and willingness to obtain within 30 days of hire

• Minimum one year of medication administration experience preferred

• Prior supervisory experience preferred

• Must be able to communicate clearly in verbal and written English

• Intermediate computer and mobile device skills
