Job summary

Direct Meds, LLC. is seeking a Wellness Coach to remotely support patients on their health and wellness journey. The role focuses on ongoing, personalized coaching after prescription, emphasizing non-clinical factors that influence progress, motivation, and long-term success.

You will guide patients from onboarding to milestone achievement and beyond, helping them maintain progress, navigate plateaus, and build sustainable habits in a fully remote setting.

Responsibilities

• Provide personalized non-clinical recommendations on nutrition, activity, routines, and habits.
• Assist patients in overcoming plateaus and barriers to progress.
• Maintain patient engagement and retention throughout their wellness journey.
• Respond to team queue tickets and complete outreach projects.
• Document session outcomes and action plans in relevant tools.

Position: Wellness Coach – Remote
Hourly Pay Range: $24–26 per hour

DirectMeds is seeking a Wellness Coach to provide ongoing, personalized support to patients throughout their health and wellness journey. Wellness Coaching serves as a key continuity‑of‑care resource for patients after prescription, helping address the non‑clinical factors that can impact progress, adherence, motivation, and long‑term success.

Wellness Coaches support patients from onboarding through reaching their goals and beyond. Rather than ending support once a patient reaches a milestone, coaches help patients maintain their progress, navigate plateaus, overcome barriers, and build sustainable habits for continued success.

What You'll Do

• Conduct scheduled one‑on‑one wellness coaching sessions, averaging approximately six sessions per day.
• Support patients throughout their full wellness journey, from onboarding and early progress through goal achievement and long‑term maintenance.
• Review patient intake forms and physical activity logs prior to sessions to prepare individualized coaching guidance.
• Provide personalized, non‑clinical recommendations related to nutrition, physical activity, routines, habits, and overall wellness.
• Help patients work through plateaus, stalled progress, weight‑loss impediments, and other challenges that may affect their success.
• Coach with a patient‑retention mindset by helping patients remain engaged and supported throughout their journey.
• Respond to team queue tickets and complete Wellness Referral outreach projects.
• Complete Wellness Reviews and conduct appropriate outreach to patients who may be experiencing little to no progress with their current wellness program.
• Request completed intake forms and confirm appointments with patients via email prior to scheduled sessions.
• Identify patients who may benefit from additional medications, supplements, or wellness solutions offered by DirectMeds and provide appropriate education regarding available options.
• Send individualized post‑session follow‑up emails outlining session highlights, successes, challenges, recommendations, and plans of action.
• Track session discoveries, outcomes, and plans of action in Google Sheets to support accurate reporting and program metrics.
• Document additional pertinent session information in Zoho Notebook.
• Collaborate with internal teams when patient needs require additional support or escalation.
• Maintain professional, compassionate, and consistent communication throughout every patient interaction.

What We’re Looking For

Required

• Strong verbal and written communication skills with the ability to build trust and rapport with patients remotely.
• Ability to provide individualized wellness guidance while remaining within the non‑clinical scope of the coaching role.
• Strong organizational skills and attention to detail.
• Ability to review patient information and translate it into actionable, easy‑to‑understand wellness recommendations.
• Comfortable discussing nutrition, physical activity, healthy habits, motivation, and lifestyle changes.
• Ability to support and motivate patients experiencing plateaus, setbacks, or stalled progress.
• Strong follow‑up and documentation skills.
• Ability to manage multiple patient sessions, outreach responsibilities, and administrative tasks throughout the day.
• Comfortable working independently in a fast‑paced, remote environment.
• Proficiency with computers, email, spreadsheets, and digital documentation platforms.

Preferred

• Previous experience in wellness coaching, health coaching, nutrition, fitness, patient support, healthcare, telehealth, or another patient‑facing environment.
• Experience supporting patients through long‑term health, wellness, or weight‑management goals.
• Experience with patient retention, engagement, or ongoing care programs.
• Familiarity with Google Workspace, Google Sheets, Zoho, CRM platforms, or similar documentation systems.
• Knowledge of wellness programs, nutritional strategies, physical activity recommendations, GLP‑1 medications, peptides, NAD+, supplements, or other wellness and longevity solutions.

Equipment Requirements

Candidates must have the following prior to their start date:

• A quality headset for virtual patient sessions and meetings.
• High‑speed internet connection.
• A professional, private workspace suitable for conducting patient sessions remotely.
