Job summary

Hendricks Regional Health is seeking a patient-centered Wellness Coach to support weight, nutrition, exercise, stress, and work-life balance goals for diverse populations including HRH associates, employer partners, and community patients.

You will develop personalized wellness plans, coordinate onsite programs, and promote a culture of wellness through collaboration with the wellness team, while maintaining confidentiality and professional standards.

Qualifications

• Bachelor’s degree in health-related field required.
• ICF-accredited coaching program (Wellcoaches) or ability to complete within first year of employment.
• 1–2 years experience required.

Responsibilities

• Assists in developing individualized wellness plans with the wellness team.
• Facilitates outpatient education in groups or individual sessions via in-person or virtual formats.
• Collaborates with HRH associates, employer partners, and patients to foster a culture of wellness.
• Communicates effectively with team members and supports program improvement efforts.
• Identifies psychosocial adaptations to illness and applies health behavior change theories.
• Applies wellness/exercise science knowledge in coaching sessions and group activities.
• Documents initial and follow-up assessments in the EMR and reports changes as needed.
• Participates in collection and reporting of program outcomes.
