Qualifications

• Bachelor's degree in a health-related field is required.
• 1-2 years experience in a related role.
• ICF-accredited coaching program or ability to complete within first year of employment.

Responsibilities

• Develop individualized wellness plans with other staff.
• Lead educational activities in groups or one-on-one sessions via multiple formats.
• Collaborate with multidisciplinary team to provide comprehensive wellness strategies.
• Communicate effectively with wellness team and stakeholders.
• Coordinate onsite wellness programs with HRH associates, employer partners, and patients.
• Identify psychosocial adaptation needs and refer as necessary.
• Apply health behavior change theories to support learning goals.
• Apply wellness/exercise knowledge in coaching populations.
• Adjust wellness prescriptions based on physician guidance.
• Assess patients pre, during, and post-exercise and refer as needed.
• Document assessment data in the EMR and report significant changes.
• Engage in CQI efforts and share resources for ethical issues.
• Participate in wellness fairs and outreach events.

Education

Bachelor's degree in health-related field
