Job summary

Hendricks Regional Health is seeking a patient-centered Wellness Coach to support weight, nutrition, exercise, stress management, and work/life balance goals for diverse populations including associates, employer partners, and community patients.

You will coordinate onsite wellness programs, work in one-on-one and group settings, and collaborate with a multidisciplinary team to promote evidence-based wellness strategies.

Qualifications

• Bachelor's degree in health-related field required.
• Certification from an ICF-accredited coaching program (Wellcoaches) or ability to complete within first year of employment.
• 1-2 years experience required.
• ICF-accredited coaching program or the ability to complete within the first year of employment.

Responsibilities

• Assists in developing individualized wellness plans with other staff.
• Participates in outpatient educational activities in small groups or individual sessions.
• Contributes to multidisciplinary team to provide wellness strategies for employer/community programs.
• Communicates effectively with wellness team and about program improvements.
• Collaborates with HRH associates, employer partners, and patients to foster a culture of wellness.
• Identifies psychosocial adaptation to illness and supports behavior change theories.
• Applies wellness and exercise science knowledge in coaching sessions.
• Develops and adjusts wellness/exercise prescriptions based on physician guidance.
• Assesses patients pre, during and post exercise and makes referrals when needed.
• Documents assessment data in EMR and reports significant changes.
