Job summary

NIVA Health is seeking a Wellness Case Manager to guide patients through their wellness journey from first interaction to ongoing programme engagement. This non-clinical, on-site role emphasizes building trust, exceptional communication, and delivering an elevated patient experience in a fast-paced clinic environment.

You’ll collaborate with providers and leadership to keep patients engaged, informed, and connected to services that support their overall wellness.

Qualifications

• High School Diploma or equivalent required
• Bachelor’s degree preferred but not required
• Experience in patient engagement, wellness services, healthcare coordination, customer success, hospitality, sales support, or account management strongly preferred
• Previous experience in a patient-facing healthcare or wellness environment preferred
• Experience with CRM systems, patient management software, or scheduling platforms preferred
• Demonstrated success in relationship-building and service-oriented environments highly desirable
• Must be available to work full-time on-site within an assigned NIVA Health clinic
• Rotating schedule may include occasional Saturdays, with a weekday off provided during those weeks

Responsibilities

What You’ll Be Doing

• Building strong, trust-based relationships with patients
