Qualifications

• Bachelor of Science degree in a related healthcare field.

Responsibilities

• Coordinate and provide care for consumers seeking treatment for medical and behavioral issues.
• Assess motivational and psychosocial issues; provide interventions as appropriate.
• Linkage to external services including government agencies, social services, and health providers per treatment plan.
• Advocate for informed decision making related to clinical status and treatment options.
• Complete forms for charting and HIPAA-compliant records for reports.
