Job summary

Transition Projects Inc. in Portland, OR seeks a Wellness Access Program Manager to lead the Wellness Access Program and supervise specialists across multiple locations.

You will coordinate with health and addiction providers to improve access to medical and behavioral health services for participants. The role requires overseeing program design, policy development, staff supervision, and collaboration with community partners to meet contract obligations.

Qualifications

• Bachelor’s degree in a related field plus 3 years in social services (1 year management preferred) or 5 years in social services (2 years management preferred).
• Experience in mental health or addiction services strongly preferred.
• Skilled in effective supervision, group leadership, and program management.
• Demonstrated ability to prepare reports and proposals, and to present such reports and proposals both verbally and in writing.
• Demonstrated ability to maintain paperwork in an organized manner.
• Strong writing abilities.

Responsibilities

• Oversee operations of the Wellness Access Program, including design, policies, procedures, staff supervision, safety, and quality assurance.
• Supervise program staff, including hiring, training, performance appraisals, and staff development.
• Develop and implement programs and policies to meet participant needs and fulfill contracts.
• Coordinate with county, city, and community partners to increase capacity and access to services.
• Maintain HMIS WellSky documentation and stakeholder reporting.
