Job summary

Transition Projects is seeking a Wellness Access Program Manager to lead and supervise services linking participants to medical, mental health, and addiction providers. You will coordinate with health partners across multiple locations and ensure program quality and compliance.

The role requires a bachelor’s degree and experience in social services, with strong leadership, written communication, and computer skills. Day shift with some weekend/evening work, salary DOE, benefits eligible.

Qualifications

  • Bachelor’s degree in a related field with 3 years of social services experience (at least 1 year management preferred).
  • Strong writing abilities.
  • Excellent computer skills including word processing, database, calendar, spreadsheet, email, Internet and graphic design programs.

Responsibilities

  • Oversee operations of the Wellness Access Program, including design, policies, procedures, staff supervision and support, safety, and quality assurance.
  • Supervise program staff, including hiring, training, performance appraisals, and development.
  • Develop and implement programs and policies to meet participant needs and fulfill contract obligations.
  • Coordinate with county, city, and community partners to increase capacity and access to services.
  • Maintain accurate program files and HMIS WellSky documentation.
