Job summary

CentraCare Health in St. Cloud, MN seeks a Nurse Practitioner or Physician Assistant to join the Weight Management team. You will work with physicians, dietitians, psychologists, pharmacists and nurses to provide patient- and family-centered care.

The role is full-time at 0.75 FTE, weekdays 7:30am-4:00pm with no weekends, nights, or holidays. Competitive salary and benefits support a comprehensive weight-management program.

Qualifications

• Graduate of an accredited Nurse Practitioner or Physician Assistant program.
• Eligibility for Minnesota licensure as a Nurse Practitioner or Physician Assistant.
• American Heart Association BLS Healthcare Provider card.
• DEA registration number prior to receiving clinical privileges.
• Minimum 2 years experience in Family Practice/Weight Management or Chronic Disease Management (preferred).

Responsibilities

• Join collaborative Weight Management team to provide patient- and family-centered care.
• Collaborate with physicians, providers, dietitians, psychologists, pharmacists and nurses.
• Support a comprehensive weight management program with varied medical options.
