Job summary

Stand Together Ventures Lab in Illinois is seeking a Nurse Practitioner or Physician Assistant to provide direct patient care in a patient-focused weight loss and wellness clinic. You will conduct evaluations, develop personalized treatment plans, and prescribe therapies within your scope.

You will monitor labs, educate patients on nutrition and lifestyle, document in EMR/EHR systems, and collaborate with a supportive team while maintaining compliance.

Qualifications

• Active, in-good-standing license and board certification.
• Ability to work independently within protocols.
• Strong communication and interpersonal skills.
• Willingness to learn new systems and treatment models.

Responsibilities

• Conduct patient evaluations, assessments, and progress visits.
• Develop and manage personalized weight loss treatment plans.
• Prescribe medications and therapies within scope of practice.
• Monitor patient progress, labs, and outcomes.
• Provide education on weight loss, nutrition, lifestyle changes, and medication use.
• Document accurately in EMR/EHR systems.
• Collaborate with healthcare staff and ensure compliance with policies and regulations.
• Support a positive patient experience.
