Responsibilities

• Supervise nursing care on the unit following policies.
• Respond to nursing and facility emergencies per policy.
• Conduct employee performance reviews, disciplinary conferences, and exit interviews as assigned.
• Assigns duties to unit staff to ensure optimal resident care.
• Conducts regular rounds and communicates findings to stakeholders.
• Work two 12-hour shifts on weekends (6am-6pm or 6pm-6am).
• Be available to work across three Chautauqua County facilities as needed.
• Weekend shift coverage with a dedicated weekend off option.
• Weekend bonus: $10/hour on top of base pay.

Heritage Ministries is in growth mode and currently seeking a Licensed Practical Nurse (LPN) Weekend Warrior to join our team!

Heritage Ministries is in growth mode and currently seeking a Licensed Practical Nurse (LPN) to join our team! The Licensed Practical Nurse (LPN) isresponsible for supervising the nursing care of residents on assigned units. They will provide appropriate evaluation of resident condition/and documents, communicating vital information to appropriate personnel. The LPN ensures the delivery of nursing care on the unit is consistent with facility standards, and established policies and procedures. Responsibilities

• Supports and promotes the nursing department functions on the unit by following established policies and procedures.
• Is knowledgeable and responds to nursing and building emergencies according to established policy and procedure. Is knowledgeable and responds appropriately to nursing/facility emergencies according to established policies and procedures.
• May serve as Assigned Supervisor for various nursing department personnel. As such, conducts employee performance reviews, disciplinary conferences, and exit interviews in a timely manner
• Assigns duties and responsibilities to unit staff members in a manner that facilities optimum delivery of care to residents.
• Conducts regular rounds on the unit and notifies appropriate personnel of findings.
• Two 12 hours shifts consisting of: Saturday and Sunday 6am-6pm or Friday and Saturday 6pm-6am
• This position supports all communities so Warriors are required to work at all three Chautauqua County facilities based on the need. However, they will be scheduled at one community for a full 12 hour shift, the shift will not be broken up to support multiple communities.
• Warriors will work 6 consecutive weekends with the 7 th off (if they choose)
• Warriors can opt to take the 7 th weekend without pay, work two days during the week for base rate of pay, or work the 7th weekend.
• This is a Part time position and not eligible for benefits.
• Weekend Bonus: Additional Ten dollars an hour!

Qualifications

• Licensed currently as a Licensed Practical Nurse in New York State in good standing. Maintenance of licensure requirements required.
• Long term care experience preferred.
