Job summary

PATH (People Assisting The Homeless) in Los Angeles, CA seeks a Substance Use Specialist to support the Metro Rail Outreach Team. The role focuses on nightly street outreach, engagement, and linking clients to housing, health and supportive services.

The position requires at least two years in substance use services and a Bachelor’s degree. Driving is required; 24 pay periods per year and weekend work are included.

Qualifications

• Two years minimum experience working directly with substance use services.
• Bachelor’s Degree in a related field is preferred.
• Must be eligible to work in the United States; PATH does not sponsor visas.

Responsibilities

• Conduct nightly outreach at Metro stations and trains to connect clients with housing and services.
• Perform VI-SPDAT assessments and document results in HMIS.
• Conduct substance use assessments and provide referrals as needed.
• Deliver street-based case management to link clients with benefits, housing and health services.
• Coordinate with partners to facilitate referrals and transportation.
• Maintain client progress documentation and follow up with supervisors.
