Responsibilities

• Examines patients/clients, obtains history, and performs tests to gather data.
• Develops patient/client diagnoses or problem lists and plans of care with measurable goals.
• Implements patient/client interventions and provides education to patients and families.
• Completes patient care documentation and coordinates discharge planning.
