Responsibilities

• Screening and evaluation of patients with physical functioning disorders and/or wound care; develop care plans.
• Deliver rehabilitative, skilled, and medically necessary interventions per standards.
• Assist with mobility, ambulation, and wound care as part of the rehab plan.
• Provide counseling to patients and families regarding physical disorders.
• Produce comprehensive discharge summaries in line with regulations and clinical requirements.
