Aperion Care in Illinois is seeking a Wound Nurse to deliver primary skin care to residents under medical direction, focusing on treatment of skin disorders. The role requires collaboration with the Director of Nursing and medical leadership to ensure high-quality wound management and resident comfort.

You will assess residents, initiate referrals, manage ulcers and infections, provide preventative care, and document outcomes to support optimal rehabilitation and wellbeing.

Qualificações

• Must be a licensed LPN or RN in the state.
• Current CPR certification required.
• Wound certification from an accredited institution preferred or required.

Responsabilidades

• Coordinate with nursing leadership on dermatologic disorders.
• Examine residents, review records, and determine when physician referral is needed.
• Provide assessment and diagnostic services using observation, inspection, and palpation.
• Initiate referrals and respond to requests from residents, physicians, or staff.
• Identify and treat skin disorders and lesions including ulcers and infections.
• Assist in rehabilitation activities and monitor therapy outcomes.
• Provide preventative skin care recommendations and education to residents.
• Document status and care plans for physicians and DON.
• Interpret policies to physicians, residents' families, and public.
• Perform other duties as assigned.
