Job summary

VitalCaring is seeking an experienced Clinical Care Manager (RN) to lead the care coordination and case management for home health patients. You will oversee the full patient journey, coordinate with clinicians and providers, and drive high-quality outcomes in a fast-paced home health setting.

The role requires active Florida RN licensure, a nursing degree, and proven care coordination or clinical leadership experience.

Qualifications

• Active RN license in Florida
• Graduate of an accredited nursing program
• Home health experience preferred
• Experience in care coordination or case management is desirable

Responsibilities

• Lead and coordinate care across the full patient journey
• Manage physician orders, documentation, and workflows in the EMR
• Partner with Intake to support timely admissions and reduce referral delays
• Conduct ongoing patient assessments and ensure plan-of-care alignment
• Lead case conferences, recertifications, and discharge planning
• Ensure compliance with clinical, state, and federal regulations
• Serve as liaison between physicians, patients, families, and internal teams
• Support in-home visits as needed
