Qualifications

Responsibilities

Provide direct patient care to short-term rehabilitation residents on our weekend shift.
Develop and implement individualized care plans in collaboration with our interdisciplinary team.
Conduct thorough assessments to identify residents' needs and goals.
Administer medications and treatments as ordered by physicians.
Monitor and report changes in residents' conditions to healthcare providers.
Collaborate with physical, occupational, and speech therapists to ensure comprehensive care.
Maintain accurate and up-to-date records of patient care and treatment plans.
