Job summary

516 Visiting Nurse Association of Wisconsin, Inc. in Milwaukee seeks a qualified Home Health Nurse for a part-time role delivering patient-centered nursing visits.

You will assess patients, implement and adjust care plans, and collaborate with an interdisciplinary team to ensure high-quality outcomes. The position is 20 hours per week, covering metro Milwaukee with weekend shifts (Saturday/Sunday 8-6), on-call nights as required, and offers a pay range of $35.50–$53.25 per hour plus benefits

Qualifications

• Active RN license in Wisconsin
• BLS for Healthcare Providers required
• Valid driver’s license
• Completion of accredited nursing program
• Typically 1 year of nursing experience

Responsibilities

• Perform all scheduled visits and update care plans.
• Assess patients physically, psychosocially, and environment during visits.
• Coordinate with physicians and RN Case Manager on treatment updates.
• Document patient records per agency, state and federal guidelines.
• Maintain knowledge of age-specific care needs and outcomes.
