Responsibilities

• Performs a comprehensive assessment on assigned patients and documents findings.
• Communicates changes in patient condition promptly to the care team.
• Develops and updates the plan of care with measurable goals.
• Identifies and prioritizes patient needs and nursing diagnoses.
• Collaborates with patient and family and other health care team members for care planning.
• Evaluates patient responses to interventions and adjusts the plan as needed.
• Assists with discharge planning and makes referrals as necessary.
• Delivers complete discharge instructions and medication reconciliation; educates patient on portal use and resources.
