The role emphasizes meaningful patient interactions, adherence to professional standards, and coordination with physicians and staff to ensure high-quality end-of-life care. Weekend coverage and on-call support are required.

Qualifications

• Must be licensed as a Registered Nurse in Virginia
• Minimum of two years of nursing experience
• Proficiency with electronic medical records (EMR) and computer-based systems

Responsibilities

• Provide holistic nursing care to hospice patients and families
• Offer on-call coverage as scheduled to ensure nursing services
• Respond to patient/caregiver calls and provide timely support
• Visit patients in home or facility to address medical and emotional needs
• Document care accurately in the electronic health record and report to IDT
