Job summary

Good Samaritan is seeking a Weekend Registered Nurse to join the Interdisciplinary Team in the Lynchburg area. You will provide holistic care, respond to crises, and support RN Case Managers with after-hours visits.

The role emphasizes hospice philosophy education, communication with physicians, and thorough documentation using EMR systems. On-site work in the Lynchburg service area is required.

Qualifications

• Virginia RN license required.
• Minimum of two years nursing experience.
• Strong communication, decision-making, and critical thinking skills.
• Proficiency with EMR and computer-based systems.
• Ability to work according to professional standards and Good Samaritan policies.

Responsibilities

• Serve as a member of the Interdisciplinary Team (IDT) to deliver holistic care to patients and families.
• Provide on-call coverage as scheduled to ensure nursing services are available to Good Samaritan patients.
• Respond to patient and caregiver phone calls and provide support for identified needs.
• Respond to medical or emotional crises and patient deaths with home or facility visits as appropriate
• Complete scheduled after-hours visits to support RN Case Managers and address patient needs.
• Coordinate care by communicating with physicians, staff, and other agencies to optimize resources.
• Educate patients and caregivers on hospice philosophy, disease progression, and the care plan.
• Monitor changes in patient and caregiver status and report to the IDT.
• Reinforce education through follow-up calls focusing on pain and symptom management.
• Document accurately using electronic point-of-care systems and report to the IDT after each shift.
