Job summary

Amedisys is seeking a Weekend RN to serve as a patient-focused case manager in home health settings. This full-time role involves 12-hour shifts on Saturdays and Sundays, plus 10–12 hours on one weekday, delivering care to patients in their homes and coordinating with the care team.

You will assess needs, develop plans of care, supervise LPNs and HHAs, provide patient teaching, and ensure timely documentation. Amedisys supports growth, flexibility, and competitive benefits.

Qualifications

• 1+ year of clinical RN experience.
• Active RN license for applicable state(s).
• Current CPR certification required.
• Valid driver’s license, reliable transportation and liability insurance.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other disciplines as indicated by patient needs or documents rationale for not doing so.
• Promotes patient health and independence through teaching and rehabilitation.
• Supervises LPNs and HHAs.
• Completes documentation timely and accurately.
• Regularly communicates patient progress to the clinical manager and care team.
• Plans and provides staff education.
• Performs on-call duties and on-call services as assigned.
• Participates in clinical development and continuing education.
