Qualifications

• One year or more of clinical experience as a Registered Nurse (RN).
• Current RN license for the state(s) you are assigned to work.
• Current CPR certification.
• Valid driver's license with reliable transportation and liability insurance.
• RN Intern program participation if <6 months experience.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other disciplines as indicated by patient needs.
• Promotes patient health and independence through teaching and rehabilitative activities.
• Supervises LPNs and HHAs.
• Completes timely and accurate documentation.
• Communicates patient progress to the clinical manager and care team.
• Plans staff education and participates in on-call duties.
• Participates in clinical development and continuing education.
